Natural Cosmetic Surgeon

Shane Sheibani
MD, FAACS, DABCS, MBA

Dr. Sheibani is the Founder, CEO and Medical Director of the New Me Surgical Institute™. He is a distinguished and prominent executive Surgeon who has performed thousands of procedures in the specialties of Advanced, Revisional and Reconstructive Cosmetic Plastic Surgery & Laser Surgery of the Face, Body and Skin. He is one of the busiest Cosmetic Surgeons in the United States with thousands of Satisfied Patients from All Over the world.

Dr. Sheibani was born in Persia in a family that valued education highly. His father was a dentist and an executive doctor, supervising hospitals, healthcare facilities and thousands of healthcare professionals for over thirty years. While Dr. Sheibani has been fascinated with art and beauty since a very young age, he became interested in the field of surgery at the age of five. Surgery has been his life passion. Matter of fact, to pursue his life passion, Dr. Sheibani came to California as a young teenager without his parents where he worked and went to school. He grew up and completed his high school studies in California.

A motivated student committed to becoming a surgeon, Dr. Sheibani was the youngest in his Medical School class at the age of nineteen. He completed Medical School at the age of twenty-three and was chosen to attend a clinical Research Fellowship at the University of Southern California.

He was admitted into a surgical residency program at an affiliated hospital of the prestigious Yale University School of Medicine, where he was trained in General Surgery.

The more Dr. Sheibani was exposed to the amazing field of cosmetic surgery, the more fascinated he became with this Super Specialty of surgery as a career. While Dr. Sheibani's training allowed him to be exposed to all the specialties of General surgery such as Orthopedic Surgery, Urology, Transplant surgery, Cardiothoracic surgery, Neurosurgery and Plastic and Reconstructive Surgery ; Dr. Sheibani realized that cosmetic surgery was where his God-Given Gifts & Talents laid. He was an artist by nature and he loved surgery and he immensely enjoyed helping otherwise healthy patients improve their lives and appearance through the performance of the best of Natural Advanced Cosmetic Surgical Procedure. That is when he was recognized by the prominent Bernard Koire, M.D. of Beverly Hills, an internationally recognized Cosmetic Surgeon with over forty five years of experience in Cosmetic Surgery of Face, Body & Skin. Dr. Koire who was one of the fore-fathers of the American Board of Cosmetic Surgery was impressed with Dr. Sheibani's commitment to the field of cosmetic surgery and his capabilities as a surgeon hence he elected Dr. Sheibani to be trained in Advanced General Cosmetic Surgery of the face, body & skin under his tutelage. Besides being a leader in the field of Cosmetic Surgery and a prominent surgeon in Beverly Hills where he served thousands of rich and famous Americans, Dr. Koire was invited to travel the world to perform surgery for the royalty in various countries. What struck Dr. Sheibani about Dr. Koire besides his unbelievable capabilities as an outstanding cosmetic surgeon was his incredible down-to-earth humility, kindness and bed-side manner as well as his devotion to his fellow man. Dr. Koire had traveled the world in various capacities over the span of half a century long career to serve the needy and the poor along with other philanthropic giants such as Mother Theresa.

After completion of this extensive training under the direct supervision of Dr. Bernard Koire, Dr. Sheibani became qualified for the American Board of Cosmetic Surgery's examination and later sat for this exam. To qualify to sit for the examination of the American Board of Cosmetic Surgery in the category of General Cosmetic Surgery there are numerous challenging requirements that a candidate surgeon must meet. The passing rate of this examination was surprisingly low (amazingly only around sixty percent of the candidates passed the exam that year) and Dr. Sheibani was able to pass this examination on his very first try. Some of these requirements included Completion of Advanced Training in Cosmetic Surgery, Demonstration of Expertise and Qualification in Cosmetic Surgery and successful completion of an oral and a written examination covering all aspects of Cosmetic Surgery of the Face, Body and the Skin. Most of the examiners of the oral examinations are leaders in the respective fields of the specialties in which they examine the candidates. (For more information regarding the examination and the required qualification for this examination please visit the American Board of Cosmetic Surgery's website at( www.AmericanBoardCosmeticSurgery.org)

In order to become a Prominent Super Specialist in Advanced, Revisional and Reconstructive Cosmetic Surgery, Dr. Sheibani's passion for this specialty led him to seek further training by Dr. Morton Mazaheri, a world renowned Cosmetic, Plastic & Reconstructive Surgeon, who had been selected by the White House to perform cosmetic plastic surgery on a First Lady.

Dr. Mazaheri - a professional sculptor, a recognized artist, a practicing attorney as well as a Double Board Certified Plastic Surgeon further trained Dr. Sheibani his beloved student & associate in the art of cosmetic plastic surgery for several years.

Dr. Sheibani's commitment to constant and never ending pursuit of excellence in education enticed him to seek further qualification in Advanced Cosmetic Surgery by the American Academy of Cosmetic Surgery. This commitment led to recognition of Dr. Sheibani as a Fellow of this Academy. American Academy of cosmetic surgery is a leading organization representing the largest group of surgeons specializing in the specialty of Cosmetic Surgery in the United States and Internationally. To be honored as a Fellow of the American Academy of Cosmetic Surgery, a surgeon must fulfill the many strict guidelines set forth by the American Academy of Cosmetic Surgery and demonstrate one's expertise and qualification by fulfilling these requirements set forth by this Academy. (For more information please visit the American Academy of Cosmetic Surgery's website at www.cosmeticsurgery.org)

Dr. Sheibani's commitment to excellence in practice of Cosmetic Surgery, and his exemplary cosmetic surgery results & patient satisfaction led to his receipt of the 2003 Best Plastic Surgeon Award of Readers' Choice Award of the Los Angeles Daily News, in the category of Best Plastic Surgeon, Runner up. This Award is granted annually to the top three Plastic Cosmetic Surgeons in Southern California by the Reader's Choice Award Committee and the hundreds of thousands of the readers of this paper.

While tirelessly working full time as the Medical Director and the senior Cosmetic Surgeon at the New Me Surgical Institute, Dr. Sheibani's insatiable thirst for knowledge and education enticed him to pursue a twenty seven month Accelerated, Advanced Graduate Education and Training in the field of Executive Healthcare Management and Administration through the University of California where he received his Fully Accredited MBA degree in 2004.

Since then, Dr. Sheibani's commitment to the advancement of the Gold Standards of the operations of the New Me Surgical Institute has led him to recruit some of the Top Executives, MBA's, Consultants and Advisors in the various healthcare management fields. In addition, he is passionately looking to recruit some of the Top Cosmetic Surgeons to join the Elite Team of Surgeons at the New Me Surgical Institute. He has personally helped thousands of patients to a better life during his career as a practicing Cosmetic and Laser Surgeon. His professional motto is "The better my patients look, the better I look!" That is why he is driven to excellence in his cosmetic plastic surgeries and pays immense attention to every detail. He is known for taking that extra time to assure that everything is done to the level of his high standards. He is a Diplomat of numerous Boards, including:

Wish to be thinner? New Me® has several options available for you…

Wish to be thinner? New Me® has several options available for you… Come in to learn if you should consider surgery and if so, what kind?

That is exactly what our Surgeons at New Me Surgical Institute can help you determine. Give us a call and arrange your free consultation. One visit can change your life.

In cooperation with our affiliates, we offer the complete gamete of Cosmetic Plastic, and reconstructive surgeries to sculpt your body and help you look better and feel better about yourself. Weight loss surgery is commonly recommended by physicians and approved for coverage by most private (PPO) insurance companies to be performed in a surgical center when the perceived risks of continued obesity are greater than the risks of surgery. In general, the risks of obesity are thought to outweigh the risks of invasive bariatric surgery when a patient is 100 pounds or more overweight or has a Body Mass Index (BMI) * of 40 or greater. However, the low risk, minimally invasive LAP-BAND bariatric procedure may be appropriate as a preventative measure at BMI less than 40.

*BMI is defined as weight in kilograms divided by height in meters squared (kg/m2).

"When the BMI is >35 and comorbidities exist, gastrointestinal surgery becomes a consideration. When the BMI is>40, surgery is the treatment of choice." from Obesity and Heart Disease. A Statement for Healthcare Professionals From the Nutrition Committee, American Heart Association

Patients who are less than 100 pounds overweight may be approved for coverage by insurance companies if they have a "life-threatening" co-morbidity such as diabetes, high blood pressure, severe sleep apnea, etc. The underlying rationale is that the overall risk of premature death is reduced by surgery for patients who meet the above criteria. However, this line of reasoning ignores the roles of preventative surgery symptom relief and improved quality of life.

Who Will Benefit the Most From Surgery?
Those patients do best who are most unhappy with their current condition and who are most willing to make a lifestyle change. Since the problem is largely genetic and at present is incurable, a life-long commitment to all aspects of treatment is necessary. Regular exercise, dietary discipline, support group participation, and occasional visits to a bariatric surgeon are important components of post-operative success.

Choosing Your Cosmetic Surgeon - By: Dr. Yoho

Choosing Your Cosmetic Surgeon - By: Dr. Yoho

There's an old expression that says, "The best way to judge a man is to meet his dog." If the individual in question happens to be a physician to whom you're entrusting your body, then this axiom would apply to his staff, his patients, his family, and anyone else who reflects his personality. If the people around him are unhappy or unhealthy, if they appear to be untrustworthy, cold, or rude, then this should give you a good indication of what the doctor is like.

A Checklist to Use During Your Consultation

- Is the surgeon a true cosmetic surgery specialist (his whole practice is "beautification" surgery), or does he do reconstructive, cancer, hands, or other kinds of surgery?
Does he claim to "operate from head to toe"? You are better off with a left-finger specialist if you need surgery on a left finger.

- What boards, if any, has your doctor passed?
Cosmetic, plastic, dermatology, and facial plastic offer board certification, but you need to look much closer, whatever the answer is. The American Board of Cosmetic Surgery (ABCS) is the only one that specifically certifies for all types of beautification surgery. Other boards are established to certify a surgeon for a more general range of tasks, and their emphasis on purely cosmetic procedures may be limited. You may call the ABCS at (708) 474-7200 to see if your surgeon has passed the cosmetic surgery boards, or is a member of another less specialized board.

- Has the doctor performed at least several hundred cases of the type of surgery you are considering?
International experts have performed many thousands of surgeries for which they are considered top-flight. Experience is much more critical than any credential or training. Medical journal studies have shown that many fancy residency programs contain very little cosmetic surgery training experience.

- Is your doctor spending his time in hospitals doing all kinds of other work or does he do his work out of an outpatient surgery facility?
Most modern cosmetic surgeon sub-specialists work almost strictly outpatient. Hospital privileges show the hospital has checked the doctor's general credentials, but never purport to certify great expertise. And if your doctor is on the staff of many hospitals, he is more likely a generalist than a cosmetic surgery specialist. Hospitals usually remove inactive doctors from their staffs, so this doctor is certainly working actively at the hospitals. All this said, however, some very competent surgeons perform cosmetic surgery in hospitals or hospital-affiliated outpatient surgical centers.

- Is your doctor involved with fellowships?
If so, beware: surgeons in training may be doing part or even most of the work.

- What is your impression of the office and the staff ?
If you have a bad impression, get out. The office and the staff reflect directly on the doctor.

- How do you feel about the doctor?
If you don't like him, leave. He should be able to make a good impression. You are marrying him in a metaphorical way when you have your surgery, and you must be willing to put up with him if you have a complication.

- Is the surgical center certified by a national organization which you can find on the Internet?
There should be a diploma somewhere in the center that you can ask to look at.

- What kind of anesthesia is recommended, and are you comfortable with the choice?
This is a complex question, and Dr. Yoho's preference is for monitored anesthesia care - a step down from general anesthesia, for the reasons cited above and on DrYoho.com: safety and comfort. And who's watching you while you are asleep?

- Speak with patients in the waiting room. What do they say?
Previous patients who are referred to you by the staff won't ever say anything bad. Every doctor, no matter who he is, has at least a few fans.

- What do the before-and-after photos look like?
If there are only a few, run. This surgeon has little experience. Be sure and ask if the photos are of the doctor's own work, and watch his response carefully. Ask the staff the same question. Believe it or not, there are sources from professional organizations that supply these photos for beginning doctors to use for "marketing." We think this is (to put the most charitable spin on it) consumer deception. In California, the law is that the doctor must tell you where the photos came from.

- Is there any part of the consultation that feels like a pressured sales job?

If so, forget it. In particular, if the doctor is trying to sell something that sounds like a one-time total makeover, run. It might be risky. Are you hassled by repeated calls from the office staff trying to get you to schedule? This is a sign of a sales and marketing organization rather than a real surgeon's office.

- Look at the doctor's biography and professional history information (CV).
Ask questions about anything you don't understand. If this isn't available, go elsewhere.

- Has the doctor published any articles in journals?
This generally lends some credibility, although there are physicians who spend more time writing and speaking than operating. While not always true, the saying goes, "Those who can, do. Those who can't, teach." And these days, the surgeon who doesn't have a lot of his work on a website for all to see may be inexperienced.

- Does the doctor or his website bad-mouth other specialties?
Remember: many specialties have contributed to cosmetic surgery techniques. Any group claiming to be the "one and only" has an aggressive marketing campaign going.

- What is the policy about touch-ups?
If the doctor has his own (certified) operating suite, touchups often can be done very reasonably, sometimes at no charge. However, if you have to go back to an expensive hospital or outpatient surgical center, the charge may be thousands of dollars even if the doctor takes no fee.

- Ask the doctor and the office staff what percentage of the patients are referred by other patients. Watch the response carefully.

You can tell when someone is telling the truth. If the doctor has been in business for years and years, and has not been able to generate a primarily referral practice, there's something wrong. Some possibilities include: the service isn't great, the surgeon isn't skilled, there's pain during the surgery, the recoveries are painful or too long, the office staff is rude, or the follow-up might be careless. A primarily advertising based practice isn't standing out above the crowd enough to become a referral-based practice.

CHOOSE YOUR COSMETIC SURGEON AS YOU WOULD CHOOSE ANY OTHER HEALTHCARE PROVIDER

Criteria for Determining a Doctor's Qualifications – Individual Character, Education, Training, Experience and Proven Competence

Choosing a doctor for any healthcare need is an important decision. Patients should choose a healthcare provider based on his or her training, education, experience and demonstrated practice history; the criteria most often used by hospitals in delineating clinical privileges. The Joint Commission on Accreditation of Healthcare Organizations (JCAHO), promotes the appropriate delineation of clinical privileges for medical staff and qualified practitioners based on training, experience, and demonstrated competence. The American Board of Medical Specialties (ABMS), supports the same. Consistently, it is the American Medical Association's policy that individual character, training, competence, experience and judgment be the criteria for granting privileges in hospitals, and that physicians representing several specialties can and should be permitted to perform the same procedures if they meet the criteria.

False Messages Misinform Patients
Consumers considering cosmetic surgery are almost universally unaware of the difference between "cosmetic" surgery and "plastic" surgery. For this reason, they are easily misled to believe board certification in plastic surgery evidences a physician's competence to perform cosmetic surgery. It does not. Importantly, with an expanding number of consumers anxious to undergo cosmetic surgery and a highly competitive pool of physicians, it is imperative consumers have access to accurate information. It is equally imperative consumers have the freedom to choose among competitors and that the number of qualified cosmetic surgeons is not artificially restricted. Unfortunately, the public almost universally equates cosmetic and plastic surgery as one and the same.

Consumers' misapprehension in this regard has them relying on incomplete and false information when choosing a physician to perform their cosmetic procedure.

Understand the Difference Between Cosmetic Surgery and Plastic Surgery
Cosmetic surgery is a subspecialty that uniquely restricts itself to the enhancement of appearance through surgical and medical techniques. It is specifically concerned with maintaining normal appearance, restoring it, or enhancing it toward some aesthetic ideal. Cosmetic surgery is a multi-disciplinary and comprehensive approach directed to all areas of the head, neck and body. Cosmetic surgery is practiced by surgeons from a variety of disciplines including board-certified dermatologists, general surgeons, oral and maxillofacial surgeons, ophthalmologists, otolaryngologists, plastic surgeons and physicians from other fields. All of these disciplines have contributed to the vital growth of cosmetic surgery. Cosmetic surgery is primarily learned during a surgeon's post residency through ongoing continuing education, training, and experience.

Unlike cosmetic surgery, plastic surgery deals with the repair, reconstruction or replacement of physical defects of form or function involving the skin, musculoskeletal system, crainomaxillofacial structure, hand extremities, breast and trunk and external genitalia. While board certification by the American Board of Plastic Surgery may evidence a physician's competence in "plastic surgery," it does not evidence competency in "cosmetic surgery" nor does it demonstrate more "cosmetic surgery" education, training or experience than that of a board-certified dermatologist, general surgeon, oral and maxillofacial surgeon, otolaryngologist, ophthalmologist or other surgeon.

Yet many board-certified plastic surgeons seek to equate the terms "plastic" and "cosmetic" in order to represent their certification in plastic surgery as evidence of their competency to perform "cosmetic" procedures. The spreading of this misconception reduces patient choice and jeopardizes patient safety. The "message" often repeated in various forums and published in advertisements by various board-certified plastic surgeons across the country falsely and deceptively informs consumers that:

- Certification by the American Board of Plastic Surgery evidences competency in cosmetic surgery

- Physicians who are not certified by the American Board of Plastic Surgery are not competent to perform cosmetic surgery

- The American Board of Plastic Surgery is the only "recognized" board with regard to the practice of cosmetic surgery

- The term "plastic surgery" is synonymous with "cosmetic surgery" or that the term "plastic surgeon" is synonymous with "cosmetic surgeon"

This misinformation has consumers believing that when searching for a cosmetic surgeon to perform cosmetic surgery, they must look only to the universe of Board Certified Plastic Surgeons and that they cannot find a competent cosmetic surgeon among boardcertified dermatologists, general surgeons, obstetric/gynecologists, ophthalmologists, oral/maxillofacial, otolaryngologists or other surgeons. (See Diagram A).

This message, which directs consumers to consider only boardcertified plastic surgeons when searching for a qualified cosmetic surgeon, is false and harms consumers in two ways:

1. Reducing Patient Choice Among Qualified Cosmetic Surgeons. There are many extremely qualified cosmetic surgeons who are not board-certified in plastic surgery. In fact, many cosmetic procedures were developed by physicians from specialties other than plastic surgery. Residency programs, whether in dermatology, general surgery, oral and maxillofacial surgery, otolaryngology or plastic surgery, do not encompass significant training in the vast array of cosmetic procedures and certainly do not provide adequate training to render a graduate prepared to practice cosmetic surgery. Surgeons develop their cosmetic surgical skills through post residency training and experience. Accordingly, to identify the most qualified surgeon for any given cosmetic procedure, one must look to and compare surgeons' post residency training, experience and practice history. Therefore, a patient may find that the most qualified surgeon available in their community to perform a breast augmentation is a board-certified general surgeon; the most qualified surgeon available to perform a chemical peel may be a board-certified otolaryngologist; or the most qualified surgeon available to perform cervicofacial liposuction may be a board-certified plastic surgeon. The point is that a cosmetic surgeon's skill and ability will depend on the surgeon's cosmetic surgery training and experience, not on his or her core board-certification. Consumers led to consider only board-certified plastic surgeons rely on irrelevant criteria and, accordingly, have their choice among qualified cosmetic surgeons greatly reduced.

2. for Their Cosmetic Surgery. A physician may complete a plastic surgery residency, qualify for the American Board of Plastic Surgery board exam, and become board-certified in plastic surgery without ever performing or receiving training with respect to many common cosmetic procedures. And, while competence in cosmetic surgery is determined by a physician's training, experience and judgment, this message has consumers believing that board-certification in plastic surgery is the golden seal when seeking a cosmetic surgeon. Bestowing expertise and competency upon physicians who may be unqualified (i.e., board-certified plastic surgeons with inadequate or no training with respect to a given cosmetic procedure), places consumers at risk.

To give a practical application of how the message impacts consumers, consider an individual seeking to undergo a blepharoplasty (cosmetic eyelid surgery) in a community where ten cosmetic surgeons are available – two dermatologists, two general surgeons, two ophthalmologists, two otolaryngologists and two plastic surgeons. The "message" proffered by many board-certified plastic surgeons would have the consumer choose from the two plastic surgeons and ignore the other eight cosmetic surgeons (see Diagram A), regardless of whether the other cosmetic surgeons may have more training, education and experience performing blepharoplasty procedures and regardless of whether the plastic surgeons may have never performed a single blepharoplasty procedure.

Information Cosmetic Surgery Patients Should Consider

Instead, the blepharoplasty patient should consider the training, education, experience and demonstrated practice history of all ten cosmetic surgeons in the area. More important, the true message all cosmetic surgery consumers should understand is that there are qualified cosmetic surgeons from various disciplines with various board certifications and there are board-certified physicians from those disciplines who are not qualified to perform cosmetic surgery.

As Diagram B illustrates, each discipline – and each depicted universe of board-certified physicians – includes qualified cosmetic surgeons as well as those not qualified. Those patients seeking cosmetic surgery who are misled to rely instead on a physician's underlying board-certification have their choice among qualified cosmetic surgeons artificially reduced and choose their cosmetic surgeon based on irrelevant information.

The total number of physicians board-certified in the named specialties (dermatology, general surgery, ophthalmology, otolaryngology and plastic surgery) exceeds 95,000, with approximately the following breakdown among the named specialties: dermatology (11,900), general surgery (49,100), ophthalmology (20,400), otolaryngology (7,000) and plastic surgery (6,600). Board-certified plastic surgeons make up only 7% of the total number of board-certified physicians.

What is the American Academy of Cosmetic Surgery?
The American Academy of Cosmetic Surgery is a professional medical society whose members are dedicated to patient safety and physician education in cosmetic surgery. Most members of AACS are dermatologic surgeons, facial plastic surgeons, head and neck surgeons, oral and maxillofacial surgeons, general surgeons, plastic surgeons or ocular plastic surgeons – all of whom specialize in cosmetic surgery. AACS is the organization that represents cosmetic surgeons in the American Medical Association through its seat in the AMA House of Delegates.

Thinking of Cosmetic Surgery? THEN, YOU MUST READ THIS!!

Thinking of Cosmetic Surgery? THEN, YOU MUST READ THIS!!

What I am about to tell you is going to probably save you a whole lot of grief and heart ache; and perhaps several revision surgeries.... Unlike what most media try to lead you to believe and make you think, most Plastic Surgeons immediately after they complete their Plastic Surgery residency hardly have had enough experience in Cosmetic Surgery to be qualified to do Cosmetic Surgery independently. Being a Board Certified Plastic Surgeon by itself alone is most likely not enough qualification to do Cosmetic Surgery. It takes surgeons years of experience of performing and managing hundreds of a particular type of Cosmetic Surgery cases to become proficient in Cosmetic Surgery of that type. Plastic Surgery residency includes a large range of diverse training, mostly non-cosmetic surgery training such as training in the treatment of injuries and complications associated with burn, accidents, cancer, skin ulcers or functional problems such as hand injuries. Most Plastic Surgeons after finishing their residency are lucky if they have done a hand full of Cosmetic Surgeries (such as Nose Jobs; Breast Lifts; Face lifts, liposuction and etc.) on their own as independent surgeons. That is why; they usually need more experience in Cosmetic Surgery. Most of them are not interested in obtaining official training in Cosmetic Surgery so they choose to learn Cosmetic Surgery over the course of years of practice, generally by trial and error.

But now (for the last several years), there is a better option for them. Only a selected few however can succeed at obtaining a position in and attend official fellowship training in Cosmetic Surgery where they can learn the true skills of Cosmetic Surgery from an expert preceptor that has been pre-qualified. Only after the completion of this Fellowship training eventually the young doctors are allowed to sit for the Board examination of the American Board of Cosmetic Surgery. The American Board of Cosmetic Surgery is the ONLY board that represents Qualified and Fellowship trained Cosmetic Surgeons (in contrast to Plastic Surgeons) who have had the experience of participating in specifically hundreds of Cosmetic Surgery cases during their training. If a surgeon is in fact a Plastic Surgeon or another Board Certified Surgeon and then completes a one or two year full fellowship training in Cosmetic Surgery and he proceeds to apply to sit for the examination of the American Board of Cosmetic Surgery, only then he may be qualified to take that examination. It is a very difficult examination and only 60% or so of the doctors pass it the first time; and it includes both a written and an oral examination. Check out the Required Qualifications of the American Board of Cosmetic Surgery for yourself and you will truly be amazed.

Most Cosmetic or Plastic Surgeons are not interested to obtain the additional required training that qualifies them to take this examination. Some organizations are fighting to take the public's attention away from the importance of this examination. www.AmericanBoardCosmeticSurgery.org is the website where you can learn more about the importance of adequate training in Cosmetic Surgery. If you are looking to have cosmetic surgery, I suggest you select a highly skilled Cosmetic Surgeon preferably one who is a Diplomate of the American Board of Cosmetic Surgery.

You know what else you MUST KNOW? After a surgeon is in fact qualified as a Diplomate of the American Board of Cosmetic Surgery, then and only then can he be qualified to apply to achieve the level and status of a Fellow of the American Academy of Cosmetic Surgery. AACS is an amazing organization with thousands of members and is the largest organization representing the specialists in the field of Cosmetic Surgery. Only a hand full of surgeons each year who are Diplomates of the American Board of Cosmetic Surgery and have completed hundreds of Cosmetic Surgeries and have successfully met the many stringent qualifications of the AACS are deemed qualified to be considered highly qualified in Cosmetic Surgery and are Bestowed the title of FELLOW of the American Academy of Cosmetic Surgery. Here is the website to check out the facts for yourself. www.cosmeticsurgery.org.

Again; I suggest you select a Cosmetic Surgeon who not only is a Diplomate of the American Board of Cosmetic Surgery but also, he has been Qualified as a Fellow of the American Academy of Cosmetic Surgery (AACS).

A Great New Me! At last!

When I embarked on the path of cosmetic surgery recently, all signposts pointed to the New Me Institute. At the age of 51, even the name conjures up an inviting image. I knew I was not alone in seeking to re-invent myself. Was NEW ME the ultimate destination? You bet!

Everywhere I looked I was bombarded both by word-of-mouth and in the print media. Inundated with positive feedback about the New Me Institute, my Internet research convinced me all the more. My original plan was a complete overhaul, not just a tune-up and lube job so to speak. It doesn’t hurt to be thorough. If my self-image has always been that of one with depth and substance, I must admit that in some areas it is only skin-deep.

The first of several scheduled procedures was an extensive facelift. More precisely I chose the deluxe model; an eyebrow lift, a facelift and a neck lift. The finishing touches were several fat transferals to get rid of those nasty Winston Churchill jowls and lines around my lips, the legacy of an ex-smoker.

If I have temporarily scaled back with my self-improvement strategy it is not because of any negative reactions or after-effects. Quite the contrary. The results from this initial surgery were so far beyond my expectations that I am content to rest on my laurels for now. Would I do it again? In a heartbeat, in the wink of a new and shapely eye.

The New Me Institute has the reputation as “the fastest growing cosmetic surgery corporation in Southern California”. Established in 1996, NEW ME, synonymous with NEW IMAGE, has served upwards of 20,000 patients since its inception.

Founder Dr. Shane Sheibani’s credentials are impressive enough to stand on their own. The roster of no less than six medical Specialty Boards with which he is affiliated such as the American Board of Cosmetic Surgery and The American Board of Laser Surgery is massive.

A kind of medical Renaissance man, the Persian born physician also maintains Fellowship status and membership in five elite societies of wider scope. He is a Fellow of the reputable American Academy of Cosmetic Surgery and the elite American Society of Laser Medicine and Surgery.

After completing medical school at the unheard of age of 23 and training in surgery at one of the affiliated hospitals of Yale University, Sheibani struck out on his own, going solo with a new vision in the business of beauty. The NEW ME dream of being “the original source for affordable, five-star cosmetic surgery” was born and subsequently flourished.

Obsessed with detail and perfectionism in his work, Sheibani covers all bases. That approach, naturally, is of instrumental benefit to his clientele. A classic overachiever, an admitted workaholic who labors virtually 24/7, Sheibani sandwiches a brief interview in an overflowing schedule of appointments. Reticent about publicizing his own individual achievements, he is careful to give credit to all members of his group, from his colleagues and his staff to the patients themselves.

When asked if he has met his intended goals for the New Me Institute, Sheibani explains, “I have very high standards. As soon as I meet one self-imposed task, some new challenge arises. It is a never-ending quest.”

The NEW ME concept is simple and direct: create a win-win situation and make it a viable option for everyone. In response to my question, “What’s special about your patients?” Sheibani smiles. He states, “What I like about the New Me patients is that they are the SMARTEST patients. They get five-star Beverly Hills quality cosmetic surgery at the most competitive prices.”

Precocious in marketing as well as his medicinal specialization, full-page color ads are a standard fixture in the most discriminating Los Angeles publications. His pat motto, “The better my patients look, the better I look!”, as all platitudes rings true.

If there are two kinds of people in the world, or at least in L.A., there are those people who believe in plastic surgery and those who cringe at the thought. Strong opinions prevail. For the rare bird that is on the fence, the New Me Institute may be just the thing.

Obviously I am in the former group, joined by those who cannot resist the latest in high-tech cosmetology. That first category also contains a specialized substratum of increasingly younger upwardly mobile professionals. The yuppie factor is a key target group of NEW ME. They revel in the vast array and up-to-date services offered by Dr. Sheibani’s baby.

In the areas of aesthetic and reconstructive surgery, two disciplines that are now both regarded as savvy rather than frivolous, the New Me Institute is in the forefront of cosmetic samplings.

My surgery has met the criteria for success on two fronts. The operations on my face were reconstructive in the sense of rejuvenating and resurrecting an ego that was rapidly deflating (although that is probably a good thing). Almost as a bonus, they were aesthetically sound. I am mesmerized by the transformation as promised.

I look much more how I feel than I have for too long a time. They don’t tell you when you are 21 (also probably a good thing) that when you are 51 your insides and outsides will be a mismatch.

I feel revived. The exhilaration at not having my face crack when I smile has not lessened since my first visit to NEW IMAGE. Not having my self-confidence go south when I look in the mirror seems to be a permanent change.

A proletariat philosophy of the god-given inalienable right to look young and be beautiful allows for the heretofore unheard of discount and highly affordable procedures. The price tags are remarkably accessible for everything from the ultra-trendy cool laser, radio-wave techniques and their dermal reconstructive abilities to the standard, meat and potatoes facelift, breast augmentations, liposuctions and tummy tucks. As a grand total, the bill does not match a traditional, tony Beverly Hills address.

In the standard one-time billing format for all cosmetic surgeries, “Dr. Shane”, as he is affectionately called, does not stint on time spent with the individual. From the initial free consultation to the aftercare visits, appointments are determined solely on an “as needed” basis rather than bowing to time constrictions.

At the main West Wilshire office, a bulky book of testimonials graces the waiting room. It is headed by an L. A. Times article on Betty Ford’s stunning facelift some 20 years ago. The inclusion highlights Dr. M. R. Mazaheri, the former First Lady’s surgeon as Sheibani’s senior associate and mentor.

More directly, it is filled with copious thank you notes and well wishers for Sheibani himself. One patient signs herself “…the new, sexy, skinny, happy…” unable to say enough. Another writes a lengthy letter lauding the “…safe, natural, relatively painless procedures”.

If I don’t actually look 21 today, I can still empathize with one gushing letter after another declaiming Sheibani’s restorative magic. My face expresses my renewed feeling of boundless energy. A satisfied customer waxed eloquent about the physician’s skills. “He could perform a facelift, blindfolded with one hand tied behind his back. He wouldn’t, but he could.”

A prime question in the New Me Institute’s patient survey concerns customer service. A premium is placed on staff decorum. That is obvious in the warm and welcoming attitude of all who greet you. If it smacks of “your service is free if we don’t-let-you-know-how-delighted-we-are-to-see-you”, all the better. In my countless post-op visits the staff never failed to notice the improvements in me each time whether minute or significant.

The various office workers are uniformly fresh and attractive. The blanket impression is good for business. Employees are usually peddling a sterling product when they act like they would work pro bono. If they have been clients of Dr. Sheibani, their presence provides invaluable, gratuitous advertising.

They are also uniformly adoring to the point of sycophancy of Dr. Sheibani’s efforts. “He is an artiste” one assistant proclaims, reassuring words to what otherwise might be a squeamish setting for a prospective patient.

All aspects of this venture have been superb and extraordinary. WOW! I say that quite sincerely. And sincerity is a main ingredient in the New Me Institute mix.

The upbeat attitude encourages the individual to improve his person. More is better. More is encouraged with discount in volume. My girlfriend, who was part of just one of NEW ME’S networks, has a mindset similar to mine. She made the comment about “buying in bulk”, an analogy fitting a town that thrives on illusion. Like me, she loves to dabble in the cosmetic palette, having already had one procedure with several more planned for the future. And she is 31.

With fantasy as its mainstay, Dr. Sheibani caters to a discerning group. Foremost, he strives to do it safely, effectively and economically. By sheer necessity, his diagnostic skills are excellent. He can usually tell at a glance what needs to be done and the optimum way to achieve it. Always current, always efficient, a patient routinely undergoes a startling satisfying metamorphosis.

If I was slightly bewildered at first by overly simplistic metaphors in explaining different surgeries, any misconceptions were quickly dispelled. The sense of genuine caring is soon evident. Not everyone lives and breathes his profession, forging ahead with single-minded purpose.

Dr. Sheibani has an exceedingly democratic desire to communicate with all who seek his services. This attitude infiltrates the atmosphere from the good doctor throughout his staff.

The New Me Institute’s networks are vast, with two branches at present in the L.A. vicinity. Sheibani’s consulting practice, however, is pervasive throughout the Southland.

Future plans include (what else?) but to expand this highly desirable franchise. For more information on the New Me Institute, log on to www.newmeintstitute.com or call 1-800 NEW ME34.

If I were you, I’d call the New Me Institute and go for it! After all you only live once, you might as well be fresh and new as you can be while in this journey we call life.

New era in cosmetic surgery...

The downing of a new era in cosmetic surgery, introducing advent of a new specialty in cosmetic surgery

There was a time when cosmetic surgery was considered exclusively reserved for the rich, famous and unusual people. Over the last several decades however this profession has evolved and grown. Millions of patients have undergone cosmetic surgery in the last several decades. According to the American Society of Aesthetic Plastic Surgeons (ASAPS), from 1997 to 2001, there was a 304% increase in the number of cosmetic procedures. To date there has been a report of over 444 percent increase since 1997. In 2002, only 6.9 million surgical and non-surgical procedures were performed in 2002. However in 2005 alone, over 10.2 million cosmetic plastic surgical procedures were performed in the United States alone, up 11 percent from 2004. In 2006, according to www.aboardcertifiedplasticsurgeonresource.com in USA alone over 11.5 million cosmetic procedures (both surgical and non-surgical) were performed. According to Dr. Mark L. Jewel, the President of the Aesthetic Society, “Statistics show interesting results in breast augmentation and breast lift” …”these areas continue to increase; breast augmentation by 9 percent and breast lift by 23 percent”.…Cosmetic Surgery profession continues to grow and evolve. Having cosmetic surgery is not merely for a select and wealthy few, but now it is an accepted and usual undertaking in almost all developed cultures globally. Baby Boomers, According to a study conducted by the ASPAS age 35 – 50 had the most procedures—44% of the total in one study. Men and women 51 to 64 had 25% of procedures; age 19-34 had 22% and age 65-and-older had 5%and age-19-and-under had 3.5%.

It is noteworthy and perhaps lifesaving to realize the quality and standard of cosmetic surgery is not the same across the globe. Every year, an increasing number of thousands of American patients travel to far away destinations across the globe including but not limited to Mexico, South America; Thailand; Dubai and Europe to have cosmetic surgery mostly to save money and sometimes to incorporate their procedures into a vacation. “Cosmetic surgery tourism is a price-driven phenomenon according to American Society of Plastic Surgeons. ASPS released a Briefing in which it cautioned the public that “it may be difficult to assess the training and credentials of surgeons outside of the United States”.

"It breaks my heart when patients who went semewhere to save money (perhaps to a foreign country with lower standards of care than those of the United States or Discount Surgical Centers with less emphasis on quality of care) come to me to fix the result of their unsatisfactory Cosmetic Surgery. Sometimes, even I, after years of experience doing some of the most difficult advanced, revisional and reconstructive Cosmetic Surgeries can't help them anymore! Please don't do this to yourself, you deserve better!

If you don't have the Time or the Money to do it Right the first time, when are you going to have the time or the money to do it all over again?"

New specialty

Cosmetic Surgery is a truly fantastic specialty. It is by far, one of the most exciting, rewarding yet challenging and subjective specialties of Medicine. There are very rare specialties of medicine that are as gratifying as cosmetic surgery in which a surgeon can participate in further improvement and transformation of their healthy and happy patients’ lives and witness the results of their efforts almost immediately. On the other hand however, Cosmetic Surgery can be a subjective specialty in that “beauty is in the eye of the beholder” hence this reality lends itself well to making the results of any cosmetic surgery subject to the judgment of the patient as well as others.

There are rare opportunities in life for any one of us to seek-out an expert who can team up with us and help transform our lives for the better. Cosmetic Surgery experts can help us achieve such transformation. Every year, around the world, millions of patients undergo successful cosmetic surgeries and find themselves closer to their ideal vision of themselves. Life is about transformation and no matter how wonderful a person may look, he or she can always improve. A patient can always wish to have more cosmetic surgery. After all, cosmetic surgery is an elective surgery and there is almost no end point; hence it can almost always be improved. The best Cosmetic Surgeons in the world only do fifty-percent of the work and are only responsible for fifty percent of the cosmetic surgery results. The other fifty percent depends on the patient’s healing capacity, genetics; medical conditions that may contribute to the results and overall multitude of factors that can not be predicted nor controlled by the patient, the surgeon or anyone else.

On rare occasions, there are those patients who are not yet satisfied with the outcome of their cosmetic surgeries and still wish to undergo revisional surgeries to improve their cosmetic surgical results. It is this reality that leading to the emergence of a new specialty within the field of cosmetic surgery.

Revising a previously performed cosmetic surgery and improving its results can be a major challenge. Those specialists who embark on performing these revisional surgeries not only must have had a proper training and adhere to the highest standards of cosmetic surgery but have performed thousands of cosmetic surgeries of the face, nose, body and breasts and skin have distinguished themselves as super specialists and seem to be getting plenty of referral for what they do best, improve the outcomes of patients’ cosmetic surgery. Cosmetic Revision Surgeons specialize in Advanced, Revisional and Reconstructive Cosmetic Surger can be called Cosmetic Revision Surgeons. Usually, in the process of the original surgery the normal anatomy has most likely been modified and the Revision-Specialist Surgeon needs to identify the normal anatomy and also be aware of the changes that can occur both as a result of normal healing process as well as the unwanted changes that can result after surgery which may be responsible for the undesired cosmetic results. As surgeons become more experienced and perform thousands of surgeries of advanced, revisional or reconstructive cosmetic surgery, their level of expertise increases in performing corrective revisional cosmetic surgery. The surgeons eventually become surgeons’ surgeons. They learn to perform those surgeries that younger and more inexperienced surgeons are not able to manage easily. They become experts and receive referrals from the community to improve other surgeons’ results.

Dr. Shane Sheibani, having performed thousands of advanced, revisional and reconstructive cosmetic surgeries of face, nose, body and the breasts is one of such surgeons. He specializes in reconstructive and advanced revisional cosmetic surgery. He has developed a reputation for himself in Southern California in this area of specialty. He is often referred patients who have had previous cosmetic surgery and the results of which are not to the patient’s satisfaction. These patients come to benefit from Dr. Shane Sheibani’s expertise and skills in order to receive the results that they are looking for.

Advanced, revisional and reconstructive cosmetic surgery is the cornerstone of Dr. Shane Sheibani’s, Practice. Dr. Sheibani is also known as Dr. New Me in association to his practice called New me Surgical Institute in Beverly Hills/Los Angeles area.. Throughout the years after thousands of cases of advanced, revisional and reconstructive cosmetic surgery and with over 15 years of education and training in medicine and surgery, Dr. Sheibani has developed and acquired the skills, techniques and technologies that allow him to perform these demanding surgeries.

The types of advanced, revisional and reconstructive cosmetic surgeries that Dr. Sheibani performs include correction of Breast cosmetic appearance after breast augmentation, breast reduction or breast lifts in addition to improvement of nasal contour; as well as facial and body cosmetic surgery improvement including but not limited to improvement of breast cleavage, breast contour deformity, areolar nipple malposition, ,poor breast contour, excess scar formation after any cosmetic surgery; nasal contour and nostril corrective surgery, face lift revision and facial contour improvement to name a few. Dr. Sheibani is a proponent of natural cosmetic surgery and has developed and acquired thousands of advanced cosmetic surgery techniques that allow him to perform a large variety of minimally invasive cosmetic surgery of the face, nose, breasts and body with minimal or no visible scars. His techniques may include hiding the incisions in the various inconspicuous areas of the body such as the skin folds, belly button; under the arms, in the hair line or utilizing the body’s own tissues for corrective cosmetic surgery and are all directed toward creating an attractive, beautiful and natural body figure with minimal evidence of surgery.

Have you had a cosmetic surgery before? Are you completely satisfied with the results of your surgery? Are you looking to improve the appearance of your breasts and create a more youthful, attractive and voluptuous appearance? Are you not happy with the results of your nose surgery? Are you not happy with the results of your tummy tuck or liposuction? Do you wish to improve the results of your face lift or other facial cosmetic surgery? Now you have a great option.

What is Natural Cosmetic Surgery?

What is Natural Cosmetic Surgery?

Cosmetic surgery is almost as much an art as it is a science. A great cosmetic surgeon must appreciate and understand the intricacies of shape, contour and symmetry, and be able to work in three dimensions. If you study the image of attractive individuals you will agree that there is clearly a certain appearance that is naturally attractive and considered shapely and beautiful. On the other hand; in the current era of plastic surgery we have learned and seen that there are also certain appearances that look un-natural; overly exaggerated or "too plastic". You may have seen those individuals who have had undeniable "Plastic Looks"; overly-stretched-face-lift-appearances or overly exaggerated body or breast contouring. Whether you are looking at attractive faces and their features; breasts, or body; there is a certain contour, symmetry, and overall appearance that reflects Natural Beauty. Even certain minimal details such as attractive dimples, a small mole or other unique features can contribute to someone's natural beauty and make their appearance and Image memorable and unique, undeniably there is a tremendous artistic ability required for the successful practice of Natural Cosmetic Surgery. A Great Natural Cosmetic Surgeon must truly observe, appreciate and understand natural beauty and enjoy paying attention to fine details. It is a natural talent and a gift for a selected few.

Over the centuries, the impression of the beauty of the human body has changed. What was considered beautiful in one era may have not been the same as another era's public's opinion of beauty. But throughout the centuries, healthy, young, fit and attractive appearances and subtly rounded curves and shapes have been associated with female beauty. On the other hand, muscular; angular, youthful, etched sculpted appearance has been suggestive of a male's masculinity. In a more intricate terms, the placement of a female brow versus that of a male; the position of the female facial features and their shape in comparison to that of a male are all noteworthy. An acceptable nasolabial angel for example for a female is different than that which is acceptable for a male. More curious is that within the range of feminine appearance for example, certain features fit certain facial or body images better or certain sizes of noses, breasts, chins, cheek bones or hips enhance the beauty of the rest of the individual's appearance. It takes a tremendous artistic strength and sensitivity to details to achieve the status of a Natural Cosmetic Surgeon.

When you look at Dr. Sheibani's before and after pictures, you realize that the results are Natural and Beautiful. It is not vividly obvious that the patients have had cosmetic surgery and the appearance of their after pictures look natural and very subtle, but certainly naturally beautiful. Natural Cosmetic Surgery is the corner stone of what Dr. Sheibani's practice is built on and Natural beauty is what he strives to achieve with excellence in cosmetic surgery. We invite you to our practice to learn first hand what Dr. Sheibani recommends for your Natural Beauty.

Non-Surgical (Knifeless) Nose Job and Facial Contouring

Non-Surgical (Knifeless) Nose Job and Facial Contouring

Fifteen minute nose recontouring procedure, known as knifeless nose job or 15-minute nonsurgical nose job. Dr. Sheibani is one of the few select cosmetic surgeons in the United States performing a new procedure for minimally invasive non-surgical nose improvements. This procedure can be performed within one office visit and has been called "the 15-minute nonsurgical nose job." This procedure, which can be performed in the office, is a new alternative for those patients who wish to have a minimally invasive procedure to improve the appearance of their nose without going under the knife for a traditional rhinoplasty. Instead of using a scalpel to cut and perform the procedure, this procedure can be performed utilizing a special filler called Restylane. It is an excellent procedure for recontouring of the nose and minimal imperfections of this area for those patients wishing to improve the shape and appearance of their nose without undergoing surgery. Through a small needle, the appropriate amount of filler is injected into the right areas of the nose and this can create an improvement in the contour of the nose and improve the appearance of this vital part of the face. It can help improve or camouflage various bumps, bulges, indentations, mal-position or poor appearance of various regions of the nose and create a better symmetry. This is an ideal procedure for patients who have had previous rhinoplasties that have left minimal or mild imperfections and for patients who wish their almost perfect Rhinoplasty to still be improved. Also, patients who have a low nasal bridge or flattening of the nasal bridge could benefit from this type of a procedure, including Asians and African-American patients, as well as some post surgical patients. The results of this type of procedure can last up to 12 months and with a minimal touch-up after this time, the patient can maintain the excellent results.

In addition to Restylane other fillers, including Juviderm or other fillers available and approved by the FDA can be injected into the desired area of the nose to help improve the nasal appearance and improve the irregularities in this area. In addition to the nose; the facial lines and wrinkles and imperfections can also be improved using these fillers. The combination of improvements of various facial areas as a result of the use of these fillers can crate a tremendously rejuvenated and naturally beautiful improved appearance. The lines extending from the nose down to the angle of the mouth (also knows as Naso-Labial Folds); the lines extending from the angle of the mouth to the chin (also known as marionette lines), the frown lines, the lines around the eyes (crows feet) and the lines around the chin as well as the contour of the cheeks and chin can all be improved using this technique. When these fillers are injected under the skin they help improve the contour of the skin. There has been definitely a shift of interest among patients over the last several years from more invasive surgery to less invasive nonsurgical alternatives. This non-invasive procedure is the ideal procedure for purposes of improving the cosmetics of the nose without any surgical procedure or intervention. The down time is very minimal, usually within a few short hours and the patient can return to regular activities and social life almost immediately. There is minimal pain and discomfort and minimal to no risk. The procedure can cost as little as one-tenth of the cost of the traditional surgeries and the recovery is ten times faster.

There are thousands of typical surgical procedures being performed for the nose and other facial re-contouring (chin Implants, Cheek Implants and facial cosmetic surgeries) on a yearly basis and this alternative technique can offer an alternative for those patients who do not wish to undergo a traditional surgical procedure. The public needs to be careful to realize that not every patient is a candidate for these procedures and there are definitely those patients who need a structural change to their nose, refinement of the cartilage and the tissues of the nose, including the bone and the cartilage of the tip and septum. As any other cosmetic surgery, there are multiple factors that a patient needs to keep in mind and consider and the best decision is always made in cooperation and consultation of an experienced Rhinoplasty surgeon.

Gender and Nasal Shape

Gender and nasal shape was evaluated and measures were obtained for the nose...

Gender and nasal shape was evaluated and measures were obtained for the nose, differences between males and females" by authors Ingo N. Springer, M.D., D.M.D., Ph.D. of University of Kiel Department of Oral and Maxillofacial and Facial Plastic Surgery and partners. This article was printed in the Plastic and Reconstructive Journal of the Society of Plastic Surgeons. According to this article, because gender specific nasal shapes are recommended for rhinoplasty, the study was setup to clarify if in fact there are truly gender related differences in the nasal area and to determine the optimal nasal shapes in the Caucasian population. According to the study, "optimal" female noses showed a horizontally and vertically lower nasion and were concave to straight in profile in the area of the dorsum as compared to "optimal" male noses which had a vertically and horizontally higher nasion and a straight profile. A supratip break was not found in any of the composites. The conclusion was that gender related differences in nasal shape appear to be subtle, nasion position being one of the main factors. A nasal hump and a supratip break are not desirable

Sheibani Cosmetic Breast Surgery Algorithm

Sheibani Cosmetic Breast Surgery Algorithm
By: Shane Sheibani, MD, FAACS, MBA

After performing thousands of breast cosmetic surgeries as well and many thousands of other types of surgeries in the last decade, I have been able to develop an algorithm that I use in order to help patients achieve the natural cosmetic surgical results they desire most while minimizing the healing time and surgical scars on the patients' breasts. I am constantly looking to improve this algorithm and any feedback or suggestions from the colleagues are most welcomed.

To get the maximum use of this algorithm; we need to keep a few very obvious but crucial points in mind:

Point 1:
After years of experience as a surgeon, I have been humbled realizing that the best surgeons in the world along with their patients as a team only contribute to fifty percent of the outcome of any surgical procedure. The other fifty percent of the outcome is dependant on variables that neither the surgeon nor the patient can control such as the patients' nature, genetics, healing capacity, the degree of immune reaction and scarring capacity of the body among many others.

Point 2:
We must keep in mind that a surgeon has to always be aware of the fact that other external variables such as family members, significant others and friends, culture, fads, marketing, advertising, entertainment and media; fashion designers and the style of clothing in fashion and regional preferences play major roles in our patient's preferences at various times. What they prefer at a certain time may not necessarily be the same as what they may want once some of the external variables have had time to influence the initial decisions of these patients. So too we must remember that every cut heals with a scar and although we may be able to improve them, at this time we can not completely erase scars once they are formed. It is always safest to do less cutting; more conservative surgery, less invasive procedures and respect the concept that we can always do more surgery later, and less is more in cosmetic surgery.

Point 3:
Cosmetic Breast Surgeries are some of the most common procedures performed in cosmetic surgery today. They are some of the most popular and most in demand procedures and really one of the safest cosmetic procedures that can be performed on an outpatient basis. While performing breast cosmetic surgery is a relatively simple and easy endeavor, the outcome of the procedure can be life changing and immensely positive if it achieves the outcome that the patient is looking to have. To achieve that end; a surgeon must clearly identify the patient's needs and desires while considering the options that are available to the patient according to her body morphology; life style; genetic limitations and social factors. Often; due to individual variables specific to the patient, the patient's desires and needs are not easily achievable. It is extremely important and crucial to plan the surgery with direct involvement and full knowledge of the patient in order to be able to successfully manage the patient's expectations of the surgery. An important credo to remember is to "under-promise and over deliver"; that is, promise much less than you know you can deliver as a cosmetic surgeon.
After years of experience performing thousands of cosmetic breast surgeries; I have come to formulate an algorithm that I am constantly looking to improve. This algorithm is intended to help the cosmetic surgeon and the patient to cooperatively decide which options fit that specific patients' needs best.

Sheibani Cosmetic Breast Surgery Algorithm :
Cosmetic Breast Surgeries involve breast augmentations with or without breast lifts (mastopexies) as well as breast reductions. Breast augmentation can generally be performed in two different ways, either by transferring fat from certain parts of the body to the breast area or by placement of implants. The Sheibani Algorithm at this time focuses on breast augmentation with implants with or without mastopexies. I am still working on expanding this algorithm in order to help it become more inclusive of breast reductions and fat transfer.
At this time, for the most part, breast augmentation is performed using an implant. While consulting with a patient; we must make several distinctions :

1) Implants: silicone vs. Saline, and Textured vs. Smooth implants; high profile vs. medium profile
2) Approaches: transareolar vs. transaxillary vs. transumbilical vs. Inframammary approaches
3) Pocket placement: under the muscle; over the muscle; under the fascia, bi-plane
4) Mastopexies: none; crescent, Circum-Areoolar/Benelli; Vertical/Key Hole technique; Vice Pattern/Anchor Lift.
5) Considering Fat transfer

The following algorithm helps us decide which patient would benefit most from fat transfer vs. implants; if we decide on implants for augmentation; which implant; which approach; with or without a mastopexy and then if the patient wishes to have a mastopexy; which type of mastopexy would best fit the patient's needs..

For Clinical decision making purposes, let's divide breasts into five Types :

Type 1: Breasts that are natural, youthful; non-ptotic, firm with sufficient breast parenchyma thickness (approximately 2 inches of thickness on pinch test). The amount of parenchyma and its consistency is important for the coverage of the implant rippling. Type one breast is for the most part the typical nulliparous breast of average patient.

Type 2: Breasts that have lost a small amount of skin elasticity with glandular pseudo ptosis and nipple/areola complex above the infra-mammary folds (no nipple areola ptosis)

Type 3: Type 2 Breasts with grade 1 ptosis

Type 4: Type 2 Breasts with grade 2 ptosis

Type 5: Type 2 Breasts with grade 3 ptosis

There are currently two types of implants available for implantation in the United States. Breast implants are either silicone or saline and either smooth or textured; high, low or medium profile.
The treatment options for Type 1 breasts is quite diverse; almost any type of breast augmentation approach including transareolar, transaxillary, transumbilical or inframammary approach; they can have any implant type; silicone or saline; they can have their implants placed over or under the breasts and they need no lift. The decision can be easily made according to the patient's preference for the placement of incisional scar. I have used all the different approaches for the placement of my implants (peri-areolar, transumbilical; transaxillary; sub-mammary) and I believe that the periareolar approach overall gives the surgeon the best visual and tactile control over the pocket and the chest wall anatomy while allowing him to create extremely naturally attractive results with minimally conspicuous scars. From the patient's point of view, the peri-areolar scar is the most versatile scar to have; since it is easily covered with clothing; usually is minimally visible after the healing process is completed; can be improved with non-invasive dermatologic procedures and can be used later for breast biopsies and breast lifts if necessary.

Patients with Type 2 breasts generally by definition have their breast gland dropping under the inframammary fold, and once the implant is placed under the muscle; if not immediately, with time the breast tissue will continue to descend downwards and eventually it will end below the location of the breast implants under the muscle and hence this breast gland position results in a higher risk of suffering from double bubble deformity. To prevent the double bubble deformity a surgeon can approach these patients with caution and consider to avoid the double bubble deformity in several different ways. If the patients have sufficient breast parenchyma to successfully camouflage the breast implant ripples, and they are willing to consider placement over the muscle; and they are willing to accept a natural breast contour, these patients can have their implants over the muscle and preferably have silicone implants used to minimize the appearance of ripples with natural minimally rippled appearance with soft contour. However; if the patient prefers an a sub-pectoral placement; then, one must either consider a mastopexy. If mastopexy scars are unacceptable to the patient depending on the grade of breast parenchyma ptosis below the inframammary fold one can either lower the inframammary fold or perform a bi-plane breast augmentation by transecting the inferior border of the Pectoralis muscle from the insertion into the skin slightly above the inframammary fold.

Type 3 same as Type 2 in addition to a crescent lift.

Type 4 same management strategy as Type 2 with a lollipop lift or a vertical lift.

Type 5 same as 2 plus an anchor lift (Vise pattern lift).
